Passport Issuing Authority and Place of Issue: What to Enter on Forms
For a U.S. passport issued domestically, enter "United States Department of State" as the issuing authority, and "United States" as the place of issue — you don't need a city or regional office name unless the specific form explicitly asks for the passport agency location.
